        • After establishing our inclusion criteria, we examined the incidence of each event over time between 2007 and 2012. We assessed rates by each year individually. These rates were adjusted for changes in age, sex, and race occurring over time using the national standard Medicare population.

        • The denominator consisted of the number of beneficiaries eligible for Medicare Parts A and B as of June 30 for each year (a mid-year denominator).
