Discharges for Ambulatory Care-Sensitive Conditions per 1,000, 2010

Clinicians have identified a group of diagnoses, such as diabetes, pneumonia and congestive heart failure, referred to as "ambulatory care-sensitive conditions." For these conditions, the rate of hospitalization is partly determined by, or sensitive to, the availability and quality of ambulatory care.
