The Geographic Query Finder allows you to locate ZIP codes and hospitals by hospital service area (HSA) and referral region (HRR). You can enter your own ZIP code or hospital name to request its HSA or HRR, or you can enter an HSA or HRR name to get a list of ZIP codes or hospitals within it. Click on the underlined text to perform a query.
ZIP code queries are based on 2005 assignments. The full ZIP code to HSA to HRR crosswalk files for 2001-05 can also be downloaded in spreadsheet format from our Download page. NOTE: These files were revised February 2007.
NOTE: The list of hospitals in our database is not exhaustive. It includes only those hospitals that had at least 80 deaths among the fee-for-service Medicare population in the period from 2001 through 2005. Smaller hospitals, and hospitals with a significant proportion of managed care patients, may not appear. Hospitals that closed during the study period will not appear. Our hospital names come from the AHA Annual Survey 2005. Recent hospital name changes may not be reflected in our data. Many hospitals are commonly known by names that are not their official names in that database.
